I'm going to go with none.

Zola and Di Mattero were around when Chelsea didn't win the league and I don't think Rossi or Macheda would have gotten a winner's medal due to lack of games for Man Utd. Blackburn was mostly British when they won it and can't think of a single Italian Arsenal player.

Yes, although players from 34 different countries have won Premier League medals, to date no Italian player has ever won one.
